Parent/Guardian Responsibility: A parent/guardian must fully complete the student's registration for swim lessons with the swim program within 60 days of the award of the scholarship or will lose the scholarship. Each parent/guardian is responsible to inform the swim program and swim instructor if there is a health issue with the student. The parent/guardian is responsible to arrange transportation to and from the lessons. Scholarship recipients shall endeavor to attend all classes during the designated class session. Failure to regularly attend may result in termination of financial assistance at SBL's discretion. The parent/guardian will follow all instructions given by the instructor/facility where the lessons are given.

Medical Requirements: Some medical conditions may require a physician or therapist note for participation in the programs. Please make the instructor/facility aware of any potential issues.

Photo/Video Release: By accepting a scholarship from SBL you are giving consent for SBL to use photographs or videos of yourself and/or the student while participating in the swimming lessons. You agree that SBL may use the photographs or videos for lawful purposes, including publication on the internet for the promotion of SBL, and you waive any right to compensation with regards to the usage of the photographs or videos.

Liability Release: I expressly acknowledge, on behalf of myself and my heirs, that I will assume the risk of any and all injuries and illness as a result of participation in the swimming lessons. I realize that SBL is simply providing financial support in order to receive self-rescue swim lessons.
